What is CHDP?
The Child Health & Disability Prevention (CHDP) Program provides complete health assessments for the early detection and prevention of disease and disability in children and youth. It is a program offered to eligible children and youth with no-cost. Children may receive well-child care visits, according to CHDP Periodicity Schedule, from birth through 20 years of age if they have Medi-Cal and from birth through 18 years of age if they are low-income with no
Medi-Cal. At our office,
however, we see patient from age 11 and up only. To shorten your waiting
time in the office, we recommend that you print the enrollment
form and the consent
form, fill them prior to your visit, and bring them with you for the first
visit. What is a CHDP health examination?
The CHDP examination is a complete head to toe health evaluation of a child or youth covering health history, physical exam, review of dental, nutritional and developmental status, screenings of hearing and vision, tests for anemia and lead exposure, immunization, and health education. Occasionally additional tests may be included. Are CHDP examinations the same as the regular check-up a child receives?
Yes, but the CHDP exam requires the provider to include all the tests mentioned in the previous question. Does the child receive the same service at every CHDP health check-up?
No. The services a child receives are based on the child's age and health status according to CHDP periodicity table. The periodicity table lists the frequency of exams, the type of tests and immunizations and also the interval between CHDP exams that are required for each age. Every CHDP provider is required to do the check-ups according to this schedule. How often can my child get a CHDP physical?
It depends upon how old the child is or if the child has managed care Medi-Cal. If the child does not have Medi-Cal managed care nor has regular Medi-Cal, he/she is eligible for physical according to CHDP periodicity table. The frequency of visits differs slightly from the frequency required by Medi-Cal managed care. What if a problem is found during the CHDP Examination?
If Dr. Lichtenstein finds a problem in any part of the exam, the child and the family may receive advice, a specific treatment (such as a prescription) or a referral for additional examinations, diagnosis and treatment. What is a CHDP referral?
Once a medical, dental, nutritional and developmental problem is identified during CHDP health exam, the child may need referral for further diagnosis and/or treatment of that problem.
Dr. Lichtenstein is obligated to make the referrals to assist the family to obtain the care their children need. How does the referral process work?
Once a child with a problem is identified through CHDP exam, Dr.
Lichtenstein discusses with the family what referral is needed. The referral is initiated at the
office. What kinds of resources are available to pay for a referral?
If your child has Medi-Cal, it may pay for the referrals. If your child does not have Medi-Cal, it may be paid by CHDTP, which is a treatment program and operates through University Medical Center (UMC). California Children Services (CCS) also provides help to children whose medical conditions meets CCS requirements. There also may be other pay sources that the child would be eligible for. CHDP nurse can help to explore all possible resources. Does the CHDP Administrative Office help with the referral process?
The CHDP staff is available to assist families and providers find and obtain needed services. A Public Health Nurse tracks children who have been identified with a problem and referred during CHDP exam. She follows up to assure that the child received the needed services. Can families independently refer their children?
It depends on the type of referral, or the type of health coverage. You can contact

CHDP at Dr. Lichtenstein's office?
All girls who have Medi-Cal ages 11-20 years, low-income girls with no health insurance ages
11-18 years who meet CHDP eligibility, foster girls, and girls who attend Head Start. Does my family qualify for CHDP?
That is determined on the basis of several factors such as your family income, number of people living in your household, your child's age, etc. Call

What does a family need to bring to a CHDP visit to confirm eligibility?
Families with Medi-Cal must bring their Medi-Cal cards, families that have no insurance must complete and sign a form declaring their income at the time of the visit.
Can I sign up for Healthy Families with you?
Yes, Dr. Lichtenstein has the applications and staffs trained to assist you complete the paperwork as well as to answer questions. Where can I go when my child needs to see a dentist while she/he only has emergency Medi-cal?
When the child has a CHDP exam and it is determined that the child needs to be referred to a dentist,
she may be referred to CHDTP at University Medical Center. Some clinics in the rural areas also offer these services at no cost or on a sliding fee. For assistance or more information call

486-8888. I don't know of any dentists that would take young children with Medi-Cal. Can you help me with that?
If your child has Medi-Cal, call us at 445-3281. We will give you names of three dentists who are currently taking Medi-Cal patients, who accept young children and are located close to your home. What if my
daughter needs a physical, but has insurance through my work?
If your insurance policy does not pay for well child check-ups, then your child may be eligible for a CHDP free exam. I do not speak English. Does the specialist have an interpreter?
Many of the specialists have an interpreter. Where is
